ADMINISTRATOR
Responsibilities:
Perform Administrative duties for the Adult Day Health Care center in accordance with State of California Title 22 regulations and established policy, program, and budget guidelines. Develop an administrative plan and procedure to insure clearly defined lines of responsibility, equitable workloads, and adequate supervision
Minimum Qualifications:
- Knowledgeable of and able to provide supervision and care of appropriate health care plans for participant receiving services
- Ability to conform to the applicable laws, rules, and regulations
- Ability to develop, implement, monitor, and oversee the daily operations and maintenance of financial and other records
- Ability to supervise, direct, and delegate the work of others
- Be of good character and hold a reputation of personal integrity
- Ability to work as a member of a team, possessing a caring and patient disposition while demonstrating a positive attitude.
- Graduation from an accredited College or University in a field related to the program and posses one of the following:
a. A Master’s Degree plus one year of successful experience in a responsible managerial, administrative, or supervisory position in a social or health service program or agency, OR
b. A Bachelor’s Degree and three years of experience in a responsible managerial, administrative, or supervisory position in a social or health service program or agency. |

PROGRAM
DIRECTOR
Responsibilities:
The Program Director shall develop, coordinate, implement and supervise all programs and services of Good Day Adult Day Health Care Center. The Program Director must be able to continuously evaluate the participant’s changing needs and adjust the program as necessary. The Program Director will supervise the employees and volunteers, and work in conjunction with the Administrator in hiring, training, and evaluating employees. The Program Director will plan and coordinate in-service training programs for the employees. The Program Director shall be at the center during all times of center operation and will act as the Administrator during periods of the Administrator’s absence. All policies and procedures of Good Day Adult Day Health Care center, applicable laws, and licensing requirements, will be followed at all times.
Minimum Qualifications:
- Qualified professional in the field of nursing, social work, psychology, or recreational, occupational, or physical therapy with demonstrated or potential competence in working with impaired/disabled or elderly individuals living at home.
- Shall be knowledgeable of physical, social, and mental health needs of the disabled and elderly population, along with being knowledgeable on programs operating within a licensed health facility or clinic.
- Ability to work as a member of a team, possessing a caring and patient disposition while demonstrating a positive attitude

REGISTERED
NURSE
Responsibilities:
- Initial and continuing assessments of each participant in the program, and development of a written plan of care in conjunction with the multi-disciplinary team: assess each participant to determine needs for medical monitoring, personal care, and assistance in activities of daily living.
- Administering medications (if needed) and monitoring of self-administration of medication(s) by participants.
- Provide health education and counseling to the participants and monitor health status.
- Maintenance of participant’s health records: Track and record participant’s health improvements.
- Provide personal care to the participants. Supervise the personal care given by program aides/medical assistants.
- Provide supervision of a maintenance program designed by the Occupational and/or Physical therapists: supervise aides or other staff that are assigned to carry out specific activities designed by the therapists to help the participants maintain current levels of functioning.
- Liaison to participant’s personal physician
- Provide emergency care to participant(s), and when needed call upon paramedics for further interventions.
Minimum Qualifications:
- Completed nursing school with a current license (RN) to practice.
- Knowledgeable of public health with reference to aging, physical assessment and care planning, health counseling, and community health services and facilities.
- Demonstrated skills and experience to assess and monitor elderly health care and conduct health education/training programs.
- Ability to work as a member of a team, possessing a caring and patient disposition while demonstrating a positive attitude.

Licensed Vocational Nurse (LVN)
Responsibilities:
- Under the supervision of the RN, administering medications (if needed) and monitoring of self-administration of medication(s) by participants.
- Under the supervision of the RN, provide health education and counseling to the participants and monitor health status.
- Under the supervision of the RN, documentation in participant’s health records: Track and record participant’s health improvements.

SOCIAL
WORKERS
Responsibilities:
- Assess the psychosocial needs of each participant and develop an individualized plan of care in conjunction with the multi-disciplinary team.
- Provide support for participants via individual and group counseling.
- Serve as liaison to participant’s family and home.
- Serve as liaison with other community agencies for coordination of services for the participants.
- Home safety evaluations.
- Implementation and coordination of the participants discharge plan.
- Develop individual service plans and evaluations of participants in the social recreational program, and work in conjunction with the Activity/Recreation staff in the delivery of activities.
Minimum Qualifications:
- A Masters Degree in Social Work from an accredited school of social work. The Social Worker shall be knowledgeable of the physical, mental, and social needs of the elderly. The professional Social Worker shall have at least two years of experience in a related area of social work, preferably in geriatrics.
- Must have oral and written skills in order to prepare and present complete and concise written materials and make presentations to the participants and/or community groups.
- Effective time management skills.
- Bilingual in Armenian or Farsi preferred, but not required.
- Ability to work as a member of a team, possessing a caring and patient disposition while demonstrating a positive attitude.
PLEASE NOTE: Applicants with a Bachelors Degree specifically in Social Work or Psychology may be considered for the position of Social Worker Assistant (responsibilities and qualifications are as stated above).

PHYSICAL THERAPIST/OCCUPATIONAL THERAPIST
Responsibilities:
- Perform an initial assessment of all participant’s physical condition and functional status; reassessments of participants as indicated
- Responsible for the development of a treatment program for participants, including setting goals
- Responsible for the treatments of participant to achieve and maintain maximum performance by using physical means such as exercise, massage, heat, etc.
- Responsible for recording and signing treatments provided to the participants
- Responsible to provide instruction and trainings to other health care personnel and family members.
Minimum Qualifications:
- Must be a licensed/registered Physical Therapist (RPT) or Occupational Therapist (OTR) within the State of California and posses current certification and readily accessible
- Some experience with the elderly and disabled population preferred, but not required.
- Ability to work as a member of a team, possessing a caring and patient disposition while demonstrating a positive attitude.

PROGRAM
AIDE
Responsibilities:
- Provides assistance to the participants during the scheduled daily activities, under direction and supervision of a professional staff member, as assigned by the Program Director (example: support participants during major group activities and recreational programs, accompany participants with program-related appointments outside of the center whenever necessary, etc.)
- Assist with participant’s personal care needs, as directed by the nurse.
- Assist with kitchen/food services during meal/snack times.
Minimum Qualifications:
- High school diploma or G.E.D. certificate. The ability to comprehend both written and spoken English; bilingual in Armenian and/or Farsi preferred.
- Some experience with the frail elderly and/or disabled individuals, either as a paid employee or as a volunteer for at least one year. This may include experience with family or friends
- Ability to work as a member of a team, possessing a caring and patient disposition while demonstrating a positive attitude.
