Step 1: The Call- your telephone call to our enrollment coordinator will start the enrollment process

Step 2: We will ask you to visit the center as a guest

Step 3: Obtaining the health record- You will be asked to provide us with your primary physician’s (and psychiatrist’s, if applicable) name(s) and telephone number(s) so we can request your medical (and psychiatric, if applicable) history and a TB clearance (please note:  TB testing can also be done by our registered nurses at the center).

Step 4: Home visit- Our social worker will schedule a home visit/safety evaluation to find out more about how we can help you or your loved one

Step 5: Review- Our enrollment coordinator will gather all the information obtained from the meetings with the family and from the physician's records and present a complete profile to our team of healthcare professionals to review the case. The team then determines whether we are the program best equipped to help you!

Step 6: Start the program- if the team agrees that our program is appropriate for you or your loved one, you will be accepted into the program.  (Please note:  If the team concludes that the services we provide are not appropriate to best serve your needs, our social workers will refer you to other agencies that will be better able to help you).
